S - c/o- Fracture of lower 1/3rd right femur shaft ..and diabetes mellitus type 2 ..with stitched wound on facet scalp .

Pt not know that he is suffered from diabetes ..but when he came in hospital for treatment then he know that he is also suffer from DM2 ..

O - appetite - normal
    Thrist - Decrease thrist ..1 to 1.5 ltr water in 24 hours ..

Urine - yellowish urine
Stool - normal
BP - 110/70 mm/hg
RR - 21/min
Pulse - 74/min
